Subpoenaed Records Request - Albany Fire Department

The Albany Fire Department is committed to assisting you with your subpoena request. Please note that reasonable fees apply for processing these records, as outlined below:

Processing Fees:
  • $24.00 per hour, per person
  • $6.00 per quarter hour or fraction thereof
  • Actual postage costs (if applicable)
Document Fees:
  • $0.10 per page for standard-sized documents (8.5 x 14 inches or smaller)
  • $0.20 per page for copies from microfilm
  • actual costs for oversized documents
  • No charge for digital files sent via email
Payment Information:

All fees must be paid no later than at the time of records delivery, per Evidence Code § 1563(b)(2).

Fire Incident Reports

Fire Incident Reports can be obtained through the Fire Department Records Department. While a written request is not mandatory, we strongly recommend submitting requests in writing to ensure clarity and expedite processing.

When submitting your request, please include:

  • Dates and times of the incident (if applicable)
  • Your relation to the property/address involved
  • Specific details about the records you're seeking

Medical Incident Records Requests

Requests for medical incident records (Fee: $0.10 per page) can be submitted in the following ways:

Required Documentation:

  • HIPAA Authorization to Release Medical Records (if the requester is not the patient)
  • Valid I.D. (e.g., California Driver’s License, Military I.D., or U.S. Passport)

If the patient’s signature is unavailable, include legal documentation such as:

  • Subpoena
  • Power of attorney
  • Letter of representation
